All employers have a moral and legal responsibility to report and investigate workplace accidents.
It also makes commercial sense as once the cause of an accident has been identified control measures can then be implemented to prevent the accident from re-occurring.

Accidents range from a minor near miss to a fatality. There are many causes to consider. Was it lack of knowledge on someone’s part? Lack of training? Their personality might mean they take risks that a more cautious person would not. Faulty equipment may have caused the accident.
An accident is, basically, an unplanned and/or uncontrolled event that led to, or could have led to injury to persons, damage to property, plant, equipment or impairment to the environment.
